Registration and Trip Forms

To register in Crew 17, print and fill out the following 4 forms. Return completed forms to the Crew 17 Advisor with $25 if not in BSA or $0 if already in BSA.

Crew 17 Annual Permission Form

Crew 17 Trip Permission Form - required for each Crew Member on each Trip.
Whitewater Challengers Waiver Form - required for every person going on the Raft Trip.
