Registration and Trip Forms

To register in Crew 17, print and fill out the following forms. Return completed forms to the Crew 17 Advisor with $25 if not in BSA or $0 if already in BSA.

• Crew 17 Annual Permission/Registration Form (due back before the end of September with the proper registration fees)

• BSA Annual Health and Medical Form (updated annually)

• Crew 17 Trip Permission Form (required for each trip)

Whitewater Challengers Waiver Form - required for every person going on the Raft Trip.
